It took some time, but with prompting from stephanj I finally found and corrected the problem that was preventing administrators from adding new servers to the page.  That should be working correctly now, without the error messages.

Regarding limitations for adding new servers (as per the other discussion), the limit is currently locked at 5% of the total servers.  We have 71 active servers, so each admin can add a total of 3 servers.  Remember this is only in effect until another solution is settled on!

I have updated the code so that if you have already reached your maximum number of servers, the 'Add new server' button will not be displayed.  Later I will add an indicator showing the total number of servers you can have when the 'add' button is not visible, but for now just keep in mind that the magic number is "3".
