We have been discussing this on IRC, but I wanted to update here as well. There were some LDAP entries deleted after performing a bare-metal server upgrade a couple weeks ago. I've been looking into it but decided that the best plan here was to just restore the data from a backup before the server upgrade occurred.
What that means is that for the server entries, everything has been reverted to its state on August 6th. If any new servers were created since then, they unfortunately would have been removed, but it also restored the older servers that had been deleted. Oddly, one of the things I noticed that all of the servers beginning with NS7... had been deleted. This might point to a code problem since the web server upgrades so I'll have to keep an eye on it.
If anyone tries to create a new server now, and it STILL seems to get deleted withing a few hours, please let me know. It's possible that the upgrades on my end or the reset of the entire servers group will fix whatever was wrong?

I'm working on upgrading the web servers this weekend and some of the code is throwing errors. I'll fix them as I have time and try to get things back online but at the moment some of the critical pages are broken for editing server entries.
