This might seem short, but I actually have another 3/4 of a page of notes putting this into more detail, and I will post it if there is interest, but here is the basics of it.

OpenNIC WHOIS Information Policy
This is the format to follow for all those participating in the OpenNIC WHOIS system. It provides all the essential information while keeping registrant privacy in mind and provides for future use such as when needing to contact a domain holder directly and issuing SSL certificates, for example.
