I made some updates to the code to ensure you can't try to save a server without those fields entered.  It's crude, just a simple warning with none of the fields saved, but it's better than the error messages you were getting before.  I'll try to add some JS filtering later.

Looking at the logs, it appears you deleted both the region and country code (the parts between "ns1" and "dns.opennic.glue") before you saved the record.  You MUST include at least a country code when creating a new server.
