So what happens if the form is there and can be submitted but internally the server is not updating zone files resulting in domains not being added/updated in DNS?

From memory I think this has happened a few times with OpenNIC tlds.

Not true. An example: Per-registrar, find the name of the input form used on the registration page. Load the page and make sure that form is there.
That not only confirms the webserver is responsive, but also confirms we are not getting an error page or an incorrect vhost. Most monitoring software allow you to specify expected page content in existing plugins, and those that don't (for the most part) allow you to write your own simple "plugin" to do the check.  It would not be a lot of work.
